Roy Crouse (Head Shrinker Extraordinaire)
"He denies suicidal ideation. He does not acknowledge symptoms of a thought disorder"....."His insight and judgment are normal."
- Determination made during a psychological evaluation of Cho Seung-Hui (Virginia Tech shooter) in December 2005.......This eveluation was certified by Special Justice Paul M. Barnett
**Uhhhhhhhhhh.......That was an expensive miscalculation.
This evaluation was ordered by a Virginia district court after Cho was accused of stalking two female schoolmates.......After being released from the psychiatric hospital it was noted on Cho's discharge summary that he "presents an imminent danger to himself as a result of mental illness."
HE WAS A DANGER TO HIMSELF! Which by default makes him a DANGER TO OTHERS! Yet he was released on the general public!! What the hell was going on?
Again, this was an expensive miscalculation. A miscalculation which cost 32 human lives........A big fuck-up indeed.**

Those are NOT judgements. Rather, this is simply a report of the client's (Cho's) own statements.
For example, if asked, you deny that you are a jackass, and if asked, you deny that you want to commit suicide, in a report I'd write: "Denies he is a jackass", Denies suicidal ideation". After all, that is exactly what you've did.
A reporting of the facts is not a judgement. The judgement comes after you've gathered the information and based on the information presented by the client (Cho) and other indications. Like past behavior, and mental state. A judgement would be based on more than what you say.
This is just information documented based on the information gathered from the person's (Cho's) report. Get it together.

Oh.. and actually, This is the JUDGEMENT that Dr. Crouse made :
Cho was transported to Carilion St. Albans Psychiatric Hospital where psychologist Roy Crouse determined that Cho "presents an imminent danger to himself as a result of mental illness."
Whoops, get your facts straight.
